Click here to find a copy of the Clinic Intake Form, fill it out, sign it and return it to the clinic by email or fax at 1-604-536-1549, then click here to pay by credit card. You will be notified when your session will take place. You may then phone the clinic at an appointed time or receive email results of your session.

The next sessions will be Wednesday, January 10, 2007 10:00 am to 12 noon and Sunday, February 25, 2007, 2:00 pm to 4:00 pm. You will be notified where the next group session will take place if the group is too big for the White Rock clinic.
